Enrollment

Please bring the documents listed below with you to your registration appointment. All documents listed must be originals. The Registrar will make copies and return the originals to you. A student will not be officially enrolled into the district until all documents are received and forms completed in Final Forms.

Required documents:

• Child's birth certificate or passport
• Parent/Guardian's driver's license or state issued ID card
• Student immunization records (these can be uploaded to Final Forms)
• Proof of Residency - you must provide TWO forms, one from each list below:
  • Signed lease agreement, mortgage statement, or property tax bill (provide one of these)
    • NOTE: if parent/guardian resides with the student in the home of another family member or friend, click the link below to download a Residency Affidavit form that will need to be completed and signed by a Notary Public. The homeowner will be required to provide an acceptable proof of residency and photo identification.
      • Dual Residency Affidavit
      • Certification of Tenancy
  • Current utility bill (gas, electric, water, sewer, waste removal; cable/phone bills will not be accepted); current pay stub, bank statement, W-2 form, voter registration, homeowner/renter/car insurance declaration (provide one of these)
• Custody decree (if applicable) - documents must be signed and stamped as filed
• Guardianship (if applicable) - documents must be signed and stamped as filed
• Special Education paperwork (if applicable) - this would include current IEP and 504 plans, and any Gifted identification information.  An official request will be made to the previous school for these documents as appropriate, however, having the documents upfront facilitates a smoother transition. Can be uploaded to Final Forms.
• Students' most recent report card (if applicable) - can be uploaded to Final Forms.

State law establishes minimum age requirements for admission to kindergarten. A child who does not meet the age requirements for admittance to kindergarten or first grade, but who will be five or six years old, respectively, prior to January 1 of the school year in which admission is requested, shall be evaluated for early admittance in accordance with District policy upon referral by the child’s parent or guardian, an educator employed by the District, a preschool educator who knows the child or a pediatrician or psychologist who knows the child. In order to enroll at Seaborn Elementary or Mineral Ridge High School, students must live within the boundaries of the Weathersfield Local School District.  (To verify, click this link to visit the Trumbull County Auditor's Property Search. Your property information will contain your school district of residence.) Students wishing to enroll in Kindergarten must turn 5 years of age by August 1.

Once you have confirmed your residence within our district, you will need to submit enrollment information to the district electronically via Final Forms. A link to Final Forms will be available in March.

Appointments will take place in the Registrar’s Office which is located in Mineral Ridge High School, 1334 Seaborn St., Mineral Ridge, OH 44440. The office is in the back of the building by the softball/baseball fields. You will enter through the back doors that face the fields. You will have to ring the bell to be let into the building.

The Mini-Rams preschool is a 5-day-a-week program for 4 and 5-year-old students. Students must be 4 years old when school starts to enroll. Click here for additional information on the program.

The Weathersfield Board of Education permits any student from any other district in the state to apply and enroll in the District free of any tuition obligation, provided that all procedures as outlined in the administrative regulations are met. Click here to read more about those procedures in our Board Policy. Open enrollment applications for new students are only accepted during certain times of the year.
